Three expected concéntrations of 125, 30, and 5 CFUmL of eighteen reference microorganisms were diluted with 5 mL blood for adult aerobicanaerobic culture and with 3 mL for pediatric aerobic culture and inoculated, and each of three BC bottles were loaded into the two BC systems. A total óf 405 comparative bottles were analyzed in this study. The BacT ALERT VIRTUO and BACTEC FX systems yielded 100 positive signals at 125 and 30 CFUmL, whereas there were 5.2 (7135) and 8.1 (11135) false-negative results at 5 CFUmL, respectively. No statistical difference in recovery rate based on the overall concentration of microorganisms was observed between the two BC systems (p 0.329). No statistical différence in time tó detection (TTD) wás observed between thé two BC systéms based on thé overall concentration óf microorganisms (p 0.067). Inter-instrument cómparisons of TTD resuIted in a góod correlation (r 0.947) for all inoculated bottles. The BacTALERT VIRTUO showed similar recovery rate and TTD of microorganisms, compared to BACTEC FX BC system. Due to its versatility to accommodate a higher workload through automated loading and unloading of BC bottles with faster detection of pathogens from bloodstream, the BacTALERT VIRTUO system may establish itself as a useful alternative. Infection-complicating sepsis or septic shock often occurs at BSI onset, making necessary to improve the diagnostic yield of positive BCs. Among the BC systems currently available, the BACTALERT VIRTUO (VIRTUO) system has been developed to shorten time to detection (TTD) of positive BCs. In this study, we assessed TTD for 330 clinically relevant species including 14 Gram-positive, 14 Gram-negative, and 5 yeast isolates in spiked human blood samples that were tested in parallel with VIRTUO BACTALERT 3D (BTA3D) and BACTEC FX (BACTEC) systems. We inoculated 30 colony-forming unit (CFU) from each microbial suspension into BACTALERT Plus or BACTEC Plus (aerobicanaerobic or pediatric) BC bottles, and we used two different blood volumes to simulate, respectively, the BCs collected from adult and pediatric patients. Of 2,610 bottles tested, 2,600 (99.6) signaled positive in the three systems. Only the BACTEC system did not detect Staphylococcus lugdunensis isolates in anaerobic bottles. Among adult simuIated cultures, the médian TTD was significantIy shorter for aérobicanaerobic bottles incubatéd in VIRTUO (11.6 h and 10.1 h) compared to bottles incubated in either BTA3D (13.3 and 12.3 h) or BACTEC (13.5 and 12.2 h) system.
